To make it easier than previous surveys, I’ve dropped the best vendor section – I’ll ask those later this year. Also, most questions list all restaurants which received votes in the last few years, and other restaurants which have recently received a lot of buzz. Hopefully this will jog your memory – but in case I’ve missed your favorite, each one has a box at the bottom which you can fill in. All votes will be counted, and if I notice a restaurant getting votes, I’ll add it to the list. You can also leave a comment on any question, which may be used in the results (anonymously). Please note the rules at the bottom of this post.
For the first time, I’ve included some restaurants outside of Portland. Feel free to vote for places on the Westside, Clackamas, or even Vancouver.

If you don’t have an opinion about every category in this survey, you can skip up to twelve questions and still be eligible for the drawing (see the fine print below).
Entries generated by script or other automated means and entries by any means which subvert the entry process are void. Multiple entries received from any person will be void. If you have not filled out at least twelve answers including the required “Best Restaurant” you will not be eligible for the drawing. Drawing winners at the email address used to enter the contest. The person receiving and replying to the winner announcement email will be considered the winner unless they specify another person within the reply email as the winner. PortlandFoodandDrink.com or the company donating the incentive will mail the contest prize to the address supplied by the recipient of the winner announcement email. Claiming of prizes requires an email response from the winning sender email address within 30 days of being notified of winning at the email address used to enter. Failure to respond shall mean that the winner forfeits the prize. PortlandFoodandDrink.com is not required to award elsewhere any prizes forfeited by the chosen winner(s).
Note that to prevent spamming, the survey can only be filled out by someone at your IP address once in two hours.
